Telehealth Notice and Consent Information

This page explains LisaCare Psychiatry and Wellness's general telehealth process. It is not a signature form and does not itself record a patient's consent. LisaCare obtains and documents any required informed consent privately through the approved clinical workflow before or at the start of care.

What telehealth means

Telehealth allows a patient and clinician in different locations to communicate using practice-approved technology. Depending on the service and technology used, an appointment may include live video, audio, secure electronic communications, or other approved methods explained to the patient through the clinical workflow.

LisaCare currently offers telehealth subject to clinical appropriateness, clinician licensure or other legal authority, and the patient's physical location at the time of the appointment. Current service states are Texas, Oregon, and Washington. The clinician will confirm eligibility.

Potential benefits

Telehealth may improve access to care, reduce travel, and allow care from a location convenient to the patient. Results and suitability vary, and telehealth does not guarantee a particular clinical outcome.

Potential risks and limitations

Telehealth may involve risks or limits, including:

  • technology, internet, audio, or video failure;
  • interruption, delay, or loss of information;
  • privacy risks if the patient or clinician cannot use a private setting;
  • limits on physical examination or observation;
  • the possibility that telehealth is not clinically appropriate for a particular concern; and
  • the need to change to another form or level of care.

The clinician may recommend stopping or rescheduling a telehealth visit or using another care option when needed for safety, privacy, technology, or clinical reasons.

Patient responsibilities

At each appointment, the patient should:

  • provide their identity and current physical location;
  • provide current contact information and, when requested, an emergency contact;
  • use a reasonably private setting and take practical privacy steps, such as headphones when appropriate;
  • tell the clinician if another person is present or can hear the visit;
  • avoid driving or other activities that make participation unsafe; and
  • follow the connection-failure and emergency instructions provided through the clinical workflow.

The clinician will tell the patient if another person is observing or participating and will obtain consent when required.

Privacy and records

Telehealth information becomes part of the clinical record as appropriate. LisaCare handles protected health information according to its HIPAA Notice of Privacy Practices and applicable law. Patients should not use the public website or general email to send clinical details, records, or urgent requests.

Any recording of a telehealth session is governed by applicable law and the private clinical consent process. Patients should ask the clinician about recording before a session begins.

Alternatives and choice

A patient may ask about the benefits, risks, and available alternatives to telehealth. Alternatives may depend on clinical need, location, provider availability, and the services LisaCare offers. A patient may decline or withdraw telehealth consent, subject to the practical effect on LisaCare's ability to provide a particular service and any care transition or safety obligations.

Emergencies and urgent needs

LisaCare telehealth is not an emergency service. If you are in immediate danger or believe you may harm yourself or someone else, call 911 or go to the nearest emergency department. In the United States, call or text 988 for the Suicide & Crisis Lifeline.

Before or during care, LisaCare may establish an emergency plan based on the patient's location and circumstances. At the start of a telehealth visit, the clinician may confirm the patient's location so emergency assistance can be directed appropriately if needed.

Connection failure

Patients will receive the practice's current reconnection or backup-contact instructions through the private clinical workflow. If a connection fails and there is an immediate safety concern, use 911 or 988 as appropriate rather than waiting for the connection to return.

Questions and documentation of consent

Patients may ask questions before agreeing to telehealth. Required consent and other compliance documentation will be received or documented through LisaCare's approved private clinical process. Do not send consent signatures or protected health information through the public website or general email.
